Back to School Forms were sent home

Teachers are sending home Welcome Packets in your student’s backpack. Please complete the paperwork and return to the main office. If you need translation help, please drop by Room 13 between 8am - 2pm.

You should have also received an email asking you to update your student’s Data Verification Form.

The information on these forms provides SPS and Hawthorne with your current contact information, student medical information and family demographics used to determine federal funding eligibility.
